The form NSO-192 (privacy act release form) is for Requesting Radiation Exposure History and the affidavit of surviving relative is for individuals requesting records for a deceased family member. The affidavit needs to be completed and notarized by the next-of-kin for a deceased family member. Please also enclose two forms of identification that includes one copy of a photo identification, such as a driver's license. Make copies to send, DO NOT SEND YOUR ORIGINALS..!!

The Defense Threat Reduction Agency (DTRA) is the Department of Defense executive Agent for the Nuclear Tests Personnel Review (NTPR) Program, which serves veterans who participated in the US atmospheric nuclear testing program.

What is the Nuclear Test Personnel Review Program (NTPR)?

The NTPR is a Department of Defense program, administered by Defense Threat Reduction Agency (DTRA), that works to confirm individual veteran's participation in U.S. atmospheric nuclear tests from 1945 to 1962, and the occupation forces of Hiroshima and Nagasaki, Japan. If the veteran is a confirmed participant of these events, NTPR provides either an actual or estimated radiation dose received by the veteran. The Department of Veterans Affairs will request this information from the Defense Threat Reduction Agency (DTRA) during VA claim reviews as required.
The NTPR Program has four elements designed to assist veterans by:
1. Providing participant and radiation dose information
2. Conducting historical records research
3. Performing veteran outreach
4. Supporting independent studies

These elements were designed to assist veterans who participated, to help the Department of Veterans Affairs (VA) and the Department of Justice (DOJ) in responding to claims and to provide veterans who are concerned about possible health effects associated with participation in US atmospheric nuclear testing.
